Due to a recent change in Pharmacy Law -- Please note the following change to our policy:

All prescriptions will require 24 hours notice. Stimulant prescriptions must be filled within 21 days. All expired prescriptions must be returned before a new prescription can be issued. There will be a $10.00 fee to re-write expired prescriptions.

  • Before completing prescription requests, all patient files need to be reviewed.
  • Please completely fill out the Prescription Request Form and send it to us by clicking on the submit button.
  • Please fill out one form for each medication and for each patient.
  • If the Prescription Request Form is NOT working on your computer you can either:
      1. Print the form and fax it to the office (210.496.2804) or
  • Prescription Requests are NOT an Emergency.
  • It is the Patient's and/or Parent's responsibility to check the status of the prescription requests.

**Please Note: Any information excluded from the form may delay the process of the prescription request. If you have any additional comments such as how the medication is working, the date of your next scheduled appointment, or why the prescription request is being submitted early, please leave this information under the comments section of the form. **

CII Prescriptions (Stimulants): Adderall, Ritalin, Concerta, Dexedrine, Dextrostat, Daytrana, etc.

These time sensitive prescriptions CANNOT be called or faxed into the pharmacy and MUST be filled within 7 days. It is important to indicate when requesting these prescriptions if the prescription will be PICK-UP or MAILED.

If you would like for us to call the prescription into your local pharmacy, please leave their contact information when requesting your prescriptions.

Here are some Helpful Hints when Refilling Prescriptions:

  • Be sure the pharmacy has ALL updated Personal and Insurance Information.
  • Turn in NEW prescriptions to the pharmacy.
  • Check prescription bottles for any remaining refills on current prescriptions.
  • Check with the pharmacy for any prescriptions that may be on "File."
  • Check with the pharmacy to see if the doctor has already called in any new prescriptions.
  • Always call the pharmacy to ensure prescriptions are ready to be picked up.
