- Payment is expected in full at the time of service
- Cash, personal checks, cashier's checks, VISA/MasterCard are accepted
- Late Cancellations and "No Show" appointments (NOT cancelled at least 24 hours in advance) CANNOT be billed to the insurance company. The responsible party will be billed in full and will be responsible for these charges. If a "Late Cancellation" or "No Show" have occurred, a credit card will be required to keep on file for future appointments.
- INSURANCE ~ Only some insurance companies will allow us to file electronically, otherwise, we provide the form for you to file. As a courtesy, we will file a claim electronically with the patient's insurance company at no charge for each visit. Any reimbursements the office receives will be endorsed over to the Insured/Responsible Party. We assume no responsibility for insurance reimbursements. Any disputes regarding charges are between the patient, or responsible party, and the insurance company. It is the responsibility of the patient and/or parent to follow-up with any insurance discrepancies. A charge may be incurred for resubmission on old claims and/or copies of old claims.
NOTE:
The doctors are not providers for any insurance companies. Please check with your insurance company about reimbursement for services. Payment is expected at time of service. The providers at this location are not accepting any new patients with Medicare, Medicaid, or Tri-care. This policy allows us to focus on what is best for our patients. Each doctor is an independent practitioner.
